Medical Plan Information
We offer four unique plans through our carrier Blue Shield of California. After 4 years of no rate changes, this year our plan premiums have undergone a slight increase.
If you do not have any changes that need to be made to your current MEDICAL plan, do not complete the enrollment or change forms in this section. You will remain on the same plan at the updated premiums.
ELIGIBILITY REQUIREMENTS
In order to enroll in a medical plan, you must be:

How do I change my plan?
You can update your medical plan by completing the form online through DocuSign:
Step 1: Enter your name and email address
Step 2: Complete the necessary fields on the DocuSign form
Step 3: Digitally sign the form and it will automatically be sent to our Benefits Administrator within our Human Resources Department
If I don't want to enroll in a medical plan right now, when is the next time I will be able to?
Don’t want to enroll right now? Not a problem. Open enrollment for medical is only during December every year. Since enrollment only happens once a year we highly recommend you consider it now! Just let your supervisor or a member of our benefits administration team know when you become interested.

Dental/Vision Plan Information
Dental and vision coverage are combined into one premium amount that is deducted from your paycheck weekly.
- Group dental coverage is provided by Principal Dental
- Vision coverage is provided by SPAN’s Self-Insured Vision Plan
Even though dental and vision coverage is combined into one deduction, claims are processed differently. Expand the “How do I make a dental claim?” or “How do I make a vision claim?” sections below for more information.

How do I make a dental claim?

How do I make a vision claim?
Vision coverage is provided by a company administered program called the SPAN Self-Insured Vision Plan. Vision claims are processed by our benefits administrator Heidi Stone by providing her with a copy of:
- the prescription and
- itemized receipts

NOTICE: There is no open enrollment for 401(k) this year as we are transitioning our 401(k) to Voya. We are excited to bring this provider to you! Please be on the lookout for communications about opportunities for education meetings in January.
401(k) Retirement Plan Information

Our company’s retirement plan can be one of the best tools available to help you build your financial future, especially if you are a new investor. Once you’ve completed the enrollment steps, you’ll be on the road to reaching your retirement goals. We want to make sure you have the retirement you deserve. So please, take the time to enroll during open enrollment in June or December!

401(k) Retirement Plan Options
See chart below for the ways in which you can contribute to your retirement plan.
Traditional: You may contribute a portion of your income to your retirement plan pre-tax. |
Roth: You may contribute a portion of your income to your retirement plan after tax (as Roth). |
Percentage: You may set up your contributions to be deducted from your weekly paycheck as a percentage (example: 7%). |
Dollar Amount: If you have a goal of contributing a specific amount to your account within the year. You may set up your contributions to be deducted from your paycheck as a flat amount from your paycheck weekly (example: $15.00). |
